SUSTAIN European Baseline Report out now

The SUSTAIN consortium is pleased to introduce you to the SUSTAIN EU Report!

Built on the early findings of the project partners, the European Report presents the fourteen integrated care sites participating in the SUSTAIN project and their characteristics. It further describes their areas for potential improvement to move towards more person-centered, safe, efficient and prevention-oriented care for older people. Stakeholders from the sites and SUSTAIN partners are jointly working on improvement projects which will be implemented and evaluated in the next phases of SUSTAIN.

The report also features a comparative analysis of the status and development of policy intentions for integrated care in the seven participating countries. It provides an overview of barriers and facilitators towards integrated care reported by stakeholders at the fourteen sites and it compares their interpretation of the SUSTAIN core domains, i.e. person-centeredness, prevention orientation, safety and efficiency.

News from the sites

In this second issue of the newsletter, we would like to focus on the first exchange between two country sites, RMZ Templin and Carework Berlin, made possible by the SUSTAIN project!

Pioneering a SUSTAIN format to facilitate learning across sites, in Germany, the RMZ Templin team had the opportunity to visit Careworks Berlin’s facilities, exchange views and explore transferability of a case management approach based on therapists, called pilot therapy.

The SUSTAIN consortium meets in Oxford

The Consortium partners will meet again between the 2nd and the 4th of November in the beautiful setting of the Wolfson College in Oxford, United Kingdom. During the meeting, organised by The International Foundation for Integrated Care (IFIC), the partners will have the opportunity to network and discuss the latest SUSTAIN project developments, as the EU Baseline Report and the sites’ data collection.

The previous meeting was held in Tallinn earlier this year, between the 20th and 22th of April 2016. For more information about the Consortium meeting in Tallinn, click here.

Sustain-eu.org translations

The SUSTAIN Consortium is glad to announce that all the web-content of www.sustain-eu.org has been translated to Norwegian, Estonian, Dutch, Spanish and Catalan. The German translation will be available soon.

SUSTAIN goes global

The SUSTAIN project keeps gaining traction worldwide and it will be featured in several important conferences at European and international level as the Annual Scientific Meeting of the Geriatric Society of America and the EUPHA Conference 2016. 

The 2016 GSA’s Annual Scientific Meeting “New Lens on Aging: Changing Attitudes, Expanding Possibilities” brings together 4,000 international experts to exchange information and to discuss the broader role of aging science. On this occasion, SUSTAIN Project Manager Simone de Bruin, together with Manon Lette (VUmc) and Annerieke Stoop (RIVM) will lead the workshop “SUSTAIN a cross European project on integrated care for older people living at home”. The session will take place on Sunday 20 November 2016 in New Orleans, Lousiana (USA), from 8.00-9.30. At the EUPHA Conference (Vienna, 10-12 November) SUSTAIN’s Norwegian sites will be presented as a poster display titled “Coordinated care initiatives in Norway and potential fields for improvement”.

The SUSTAIN project and its sites will be also featured in the context of key regional and national conferences, as the Berlin LTC Conference (9 November 2016), where Sophia Schlette (Stiftung Gesundheit) will be joined by the CEOs of KV RegioMed Zentrum Templin and Pflegewerk, SUSTAIN’s German sites.
