Primary care settings are optimally positioned to address the initial palliative care needs of older adults with serious or complex illness. However, many providers have had little or no training on how to have a conversation about end-of-life care and are uncertain about the new reimbursement rules governing these types of conversations. At the completion of this 45 minute webinar participants will be able to:

  • Identify barriers to having end-of-life conversations with patients

  • Describe best language for having these conversations

  • Identify a multidisciplinary team approach to overcome barriers

  • Describe billing codes and how to be reimbursed for end-of-life conversations
