2016-2017 School Calendar
Thank you so much for following our carpool procedures. If you have any questions about carpool please check the website. Remember to never drop off in the front loop! 

 Middle School students may get their first 2 sweatshirts for the discounted rate of $10.00. After that the sweatshirts go to the standard $25.00. Orders must be placed by the 26th of August to receive the discounted price. 
Hoodies sold online or at the front desk.
Note: NO OTHER SWEATSHIRT CAN BE WORN BY MIDDLE SCHOOL STUDENTS OTHER THAN THESE SCHOOL ISSUED SWEATSHIRTS.

To apply for free and reduced lunch log on to your SIS account. All applications are now done online. 
BALLROOM DANCE CLUBS
KIDS CLUB: Grades 3rd-4th
BALLROOM CLUB: Grades 5th-6th
DATES:  September 12th - November 14th
TIMES:  Rehearsals on Mondays at Lakeview from 
3-3:45pm (Ballroom Club) and 3:45-4:30pm (Kids Club)
COST:  $40 fee due with registration form at front office.
Space is limited, forms and fee due by September 3rd.

SIS is our student information system. All parents need to set up an account to track grades/homework/add emergency contacts/update info.

Follow the instructions listed in the link below and earn one hour of volunteer time per family PLUS find out who your students teacher is on or after August 3rd!  

If you have never set up an SIS account please do so. If you need your student numbers contact the front office. 

**The front office will not give out teacher information over the phone.**

Parents the STEM lab needs your help!  We need building materials.  If you can, please send any of the following recycling items with your student to school. We need lots of stuff since we service 600 elementary students!

Lakeview Academy's State Championship Cross Country is ready to begin practicing for the new season!
Grades 6-9
Our first practice will be held on August 22nd at Neptune Park. We Practice from 3:15-4:15 on Monday's, Tuesday's and Thursday's, and from 12:20-1:20 on Friday's! PALS I and II Training
ALL volunteers working at the school must be at least PALS I trained and have a volunteer badge.
 
PALS I and PALS II Training booklets and quizes available at the front office. 
Lakeview Academy does have the ability to take fingerprints and submit background checks. 
 
When doing Pals 1 and Pals 2 please do not bring children

PALS 1
Volunteers at PALS I will have a significant level of interaction with students, yet will still be supervised by the classroom teacher. Their main responsibilities are to assist with small group instruction in math, language arts or any of the additional core subjects as needed. Volunteers at this level would be asked to go through some basic training.
 
PALS 2
Volunteers at PALS II  will have the greatest interaction with students and could potentially be left unsupervised with students. However, they will not be permitted at any time to be alone with any one student. More than one student or another teacher must be in close proximity (for instance, the volunteer may work with a student in the hall -- a public thoroughfare -- with the classroom door remaining open). They could provide the same assistance as a Level One volunteer. Yet the main responsibilities of these volunteers include: after school tutoring, working with students in class who need individual assistance, substitute teaching, and chaperoning field trips. Training would be required for volunteers at this level. 
Volunteers at this level will be required to be fingerprinted and have a background check in accordance with Utah Law 53A-3-410. Volunteers whose background check shows convictions for any class A misdemeanor or felony, including pleas in abeyance and diversion agreements(or the equivalent from another state) will not be permitted to be a PALS Two volunteer however; they may be allowed to be a PALS One volunteer. These background checks must be done every four years at the volunteer’s expense. Complete detailed information regarding these procedures is available upon request.
